What the video shows
When a patient sends in their medical history form, you can add their answers, and their signature, to their medical history in one step. This video is for whoever checks the forms that come in.
Forms that patients send in are under Patients, then Mail Shots, then Patient Forms. Here's the list of your Patient Forms mail shots.
Double-click a Patient Forms mail shot to open it. It lists your published forms.
Right-click your medical history form, and choose Show Patient Responses. A new tab opens with the forms sent in.
Click the form your patient sent. It came from the general link, so it isn't linked to a patient yet. Their answers are on the right.
Press Add to medical history.
First, choose who it belongs to. Start typing their surname, then pick the right patient from the list.
You'll see every question, with the patient's answer and any details, and that the form was signed.
Anything they left blank is greyed out, and skipped.
Tick which Yes answers should also become alerts on their record. Warnings and hazards are ticked for you. Here, we tick asthma as well.
Press Add to medical history. A message confirms it's been added, and how many alerts were added.
Now open the patient's record. Find them as usual, here by surname, open their details, and go to the Med. History tab.
Click the new medical history. There's their signature, and every answer they gave.
And the answers you ticked are now active conditions for this patient. A warning, like the penicillin allergy, pops up whenever their record is opened.
