Patients can fill in their medical history online before they come in. You build the form once in the Form Designer, share its link, and add each completed form to the patient’s medical history with Add to medical history. This arrived in Release 2.0.65.
These three short videos take you through it, in order.
- Building an online medical history form (1:42) – Build a medical history form in the Form Designer: add Medical condition questions, use Insert medical history questions, add a signature, then save and publish it to get a web address to share with patients.
- How patients fill in your medical history form online (1:03) – See what your patients see when they open your online medical history form: Yes/No questions, details boxes, signing and submitting, with nothing to log in to.
- Adding a patient's online form to their medical history (1:53) – Take a medical history form a patient has sent in, match it to the right patient, choose which answers become alerts, and add it with their signature to their medical history in one step.
Good to know
- Nothing goes on a patient's record until a member of staff checks the form and presses Add to medical history.
- A condition with no patient wording set in Housekeeping is asked by its name.
- Questions the patient left blank are greyed out and skipped.
- Yes answers to Warning and Hazard conditions are ticked to become alerts; you can tick others too.
- Adding a new medical history makes it the patient's current one, in place of their earlier ones.
