What the video shows
This is what your patients see when they open your medical history form. It's useful to know, so you can help them if they ring with a question.
They open it from the link you've sent them. There's nothing to log in to, and nothing to install.
Each question is a simple Yes or No. A red star means the question must be answered.
When they answer Yes, a box opens, so they can tell you more.
The details are optional, but they help you.
Here's another Yes, with a few words about it.
Most patients will answer No to most questions. A question that doesn't have to be answered can be left, like Angina here.
They sign in the box, with a finger, or with the mouse on a computer.
Then they press Submit Form.
Their answers come to the practice. Nothing goes on their record until one of your team checks it, and adds it.
