What the video shows
You can ask your patients to fill in their medical history online, before they come in. This video shows how to build that form. It's for whoever looks after your practice's forms.
Go to Utilities in the menu on the left, then Form Designer. It opens on your list of forms.
Press New Form, and choose Standard form. Your questions will be set out one under another.
Give it a name your patients will recognise, such as Medical history, and a short description.
From the list on the left, drag a Medical condition question onto the form.
On the right, choose which of your medical conditions it asks about. The question fills itself in, from the wording in your medical conditions.
You can change the wording. A Yes answer opens a box for more details, and you can say what to ask for. Tick Required if they must answer.
To add all your usual medical history questions at once, press Insert medical history questions.
Asthma is now asked twice. To remove a question you don't want, press its red cross.
A condition with no patient wording is asked by its name, like Angina here. You can change its wording in the same way.
Finish with a signature. Drag it to the end of the form, so your patient can sign it.
Press Save. A message tells you the form has been saved.
When it's ready, press Publish. Your form now has its own web address. Copy it, to share with your patients.
Close this. Back on the Forms tab, you'll see your form is published, ready for your patients.
