To date, the additional information for the FP17DCO form was derived from the appointments General Section.
Going forward, the information is derived from the FP17DCO dialog.
The Additional Information text input now allows the clinician to enter the required text.

The acceptance date was incorrectly formatted, meaning the 20 of 2026 was shown and not the 26.
